Submissions
Online Submission: The MED now offers options for online submission of applications using your computer, phone, or another device with internet access.
- Please note that online submission requires use of third-party fingerprint services. See “Third-Party Fingerprint Services” below for additional guidance.
Other Methods: Please do not submit multiple of the same application[s] as this may complicate the application process and significantly delay your application processing time. For example, please do not submit an application online and make an in-person appointment.
Drop-off (Lakewood Office only)
You can drop off your application at the MED’s Lakewood Office drop box. The Lakewood office’s drop box is located in the lobby and is available only from 8 a.m. to 5 p.m. Monday through Friday. When submitting via drop-off, please ensure your complete application is in an envelope to keep the application, associated documents, and payment of fees intact.
- Please note that drop-off submission requires use of third-party fingerprint services. See “Third-Party Fingerprint Services” below for additional instructions.

Schedule an appointment
This scheduling tool will allow you to choose your appointment type and the closest office to you. You may choose to have your fingerprints taken during your in-person appointment or can use third-party fingerprint services (see additional guidance below on third-party fingerprint services). Having fingerprints taken at the MED will cost an additional $39.50. Payments can be made by check, money order, and credit/debit cards. No cash will be accepted. See the MED Fee Schedule for associated application and licensing fees.
Third-Party Fingerprint Services
Customers submitting their application online, through the mail and via drop-off MUST use third-party fingerprint providers. Customers can visit our website to find information on the available third-party fingerprint service providers.
You must submit your application PRIOR TO being fingerprinted by a third party. If you are submitting your application online, via USPS, or dropping off your application, please have your fingerprints taken by the third party AFTER submitting your application (preferably within 7 days of submission).
Issuance of Licensing Badge
When the MED is prepared to issue your owner or employee license (badge), it will be mailed to the address listed on your application - you cannot pick up your badge.
