MED Health and Safety Advisories

If the MED identifies evidence of contaminants in regulated marijuana or other public health and safety risks to consumers, the MED may issue a Health and Safety Advisory (see below) to warn the public not to consume and to dispose of any contaminated product they may have purchased.

Levels IV Inc and SKL Holdings III LLC
 

Jan. 13, 2025- The Colorado Department of Revenue (DOR), in conjunction with the
Colorado Department of Public Health and Environment (CDPHE), is issuing a Health and
Safety Advisory due to the identification of contaminate and potentially contaminated Regulated Marijuana
Flower (bud/shake/trim) produced by Levels IV Inc and SKL Holdings III LLC.

The CDPHE and DOR deem it a threat to public health and safety when marijuana is found to
have Total Yeast and Mold above the acceptable limits established in Colorado Marijuana
Rule 4-215. This advisory includes Harvest Batches tested and found to have exceeded
acceptable limits established for Total Yeast and Mold as well as Harvest Batches that were
produced at the same time and location as the contaminated batches, identified below as
potentially contaminated.

The affected product is listed below by license number and Harvest Batch and was sold
between December 28, 2023, and December 11, 2024. 

Consumers who have this affected marijuana in their possession should destroy it or return it to
the Regulated Marijuana Store from which it was purchased for proper disposal. Consumers
who experience adverse health effects from consuming the Regulated Marijuana should seek
medical attention immediately and report the event to the Marijuana Enforcement Division by
submitting a MED Reporting Form. 

Individuals who experience adverse health effects after consuming the affected product should seek medical attention immediately and report the event to the MED by submitting an MED Reporting Form.
